When is the deadline for nominations?
For FY19, the deadline is November 30, 2018. Nominations submitted after this date will be held for consideration in FY20 (July 1, 2019- June 30, 2020).

What do I need to submit a nomination?
You must complete the official online nomination form and submit a letter of nomination and a nominee’s professional bio to alumni@bsd.uchicago.edu.

How long will the form take?
Completing the online nomination form takes 20-30 minutes on average. To preview the form before you begin, click here.

Can we start the online nomination form and return at a later time to complete it?
Yes, your progress will be saved once you start the online nomination form. Please note: you must use the same device each time to have your progress saved.

What should my letter of nomination address?
A letter of nomination is also required to support your candidate for the Distinguished Service Award. You are requested to provide a narrative specifically addressing: the nominee's achievements and breadth of impact on the life sciences ranging from laboratory research, clinical care, health service administration, and efforts leading to improved public health. Please specifically describe why this nominee meets the qualifications for the Distinguished Service Award and represents the values and priorities of the University of Chicago.

To help make the evaluation provided as similar in content and focus as possible, please consider the criteria and considerations indicated below:
1) The significance of the body of work,
2) The overall strength of the nominee in the area(s) being presented, and
3) The innovation of the nominee's work and the impact of this work in the alumnus' area of focus.

The latter factors should all be addressed, however, please give special attention to innovation and impact. Has the nominee moved current research, public health, patient care, or other practice areas forward in a significant manner? This letter will serve as "your case" to the Alumni Awards Committee.

Can I nominate myself?
Yes, the Alumni Awards Committee accepts and encourages self-nominations.

What is the Distinguished Service Award for Early Achievement?
The Distinguished Service Award for Early Achievement requirements include the same criteria as the Distinguished Service Award, but for alumni who have graduated within the past fifteen years.

Who is eligible for the Distinguished Service Award?
Alumni who earned a degree from the Pritzker School of Medicine or the Division of Biological Sciences are eligible to be nominated for the Distinguished Service Award. Those who earned an undergraduate degree in the sciences are not eligible for awards from the MBSAA, but should consider the awards presented by the University of Chicago Alumni Association.

When will the recipients be announced?
The Distinguished Service Award recipients will be notified in early 2019.

When will the Distinguished Service Awards be honored?
Recipients will be honored at the Distinguished Service Awards Luncheon during Reunion weekend.

How many people receive a Distinguished Service Award each year?
In accordance with the bylaws of the MBSAA, each year, no more than 3 Distinguished Service Awards and 1 Early Achievement will be awarded, except in special circumstances and with the approval of the Executive Committee.
